Getting started is easy! Just call one of our Information Specialists at (719) 572-6100 to begin enrollment Monday through Friday from 7 a.m. to 7 p.m. or 8 a.m. to 4:30 p.m. Saturday and Sunday. Medicaid and most 3rd party insurances are covered.

In addition, Standard Case Management assists clients in learning daily living skills that will lead to more independent living.

Telephonic care provides telephonic support to help people better manage their self-care to improve their overall physical and mental health. Health conditions supported include asthma, chronic pain, congestive heart failure, diabetes type II, heart disease, depression and sleep issues. TeleCare also supports those in recovery from substance abuse.
