Peak Vista Partnership and other co-locations
Recognizing the importance of overall health and wellness, an integration of primary and mental health care was implemented in partnership with Peak Vista Community Health Centers and other area primary health care providers.
Peak Vista patients and Medicaid clients of other select primary care practitioners who are struggling with mental health issues can obtain short term therapy at the following medical clinics: Family Health Clinic, Myron Stratton, Divide, Homeless, Fountain Clinics, and Dr. Johnson's office.
Eligibility Criteria:
Referral must come from a Peak Vista medical provider.
Insurance(s) Accepted:
A Collaboration with Peak Vista, no charge to clients for co-visits and $5 for a follow-up.
