Community TIES

Adults with co-occurring mental illness and developmental delays can have multi-dimensional, critical needs. Community TIES works through an Intensive case management and consultation model for clients in need of additional specialized population, short-term supports to ensure successful community living.

Community TIES provides support to caregivers, service providers, families and guardians or people with co-occurring mental illness and developmental disabilities. The purpose of this program is to provide training, consultation and support for positive outcome to maintain community supported living. The Community TIES Program consists of a comprehensive and structured program driven by the client’s individualized treatment plan.

 
Services Offered:
Training - On a variety of topics related to development disabilities for caregivers, case managers, consumers and their families and guardians. All trainings and treatment are based on evidence-based materials. Interventions are based on principles from: Illness management and Recovery, Cognitive Behavioral treatment, Motivational Interviewing, and Positive Behavioral Support.
Intervention - Consultation to prevent and address challenging behaviors, to support community placements, and to help foster recovery, self efficacy and independence
Evaluation – To assess, triage, screen, and refer to appropriate services.
Services – Intensive case management and consultation for client open to PPMH in need of additional short term supports for successful community living.

 

Eligibility Criteria:
Anyone with a co-occurring mental illness and developmental disability qualifies for Community TIES. Longer term intensive case management services are available if the clinical need is demonstrated. Referral can be made by primary therapist using consultation process. External referrals can be made through Admissions or by contacting staff directly.

One of the largest nonprofits in Colorado Springs, AspenPointe traces its roots back to 1875 when we were called the Springs Relief Society. That organization, which delivered coal, lumber and clothing to the needy, was the genesis of what today is a comprehensive and integrated system of care that addresses the overall wellness of each client whether they need behavioral health services, counseling services or career services.

We have 12 organizations that serve more than 30,000 individuals and families each year through behavioral health services, counseling services, career services, education, housing, jail diversion/reintegration, telephonic wellness and care management.

We collaborate with clergy, educators, employers, other health and human service agencies, the legal/criminal justice system, other behavioral health providers, insurers, and many other organizations to leverage our collective resources in order to better our communities and the lives of its residents.

We are a 501(c)(3) nonprofit funded largely by Medicaid, state and federal grants, local government contracts, and generous donations.
