Peer Navigator
We offer veterans and their families an individual peer navigator who helps them plot a course through the sometimes overwhelming challenges of day-to-day life and the numerous, complex systems available to support them.
Enterprises
Competitive business practices to solve some of our communities’ most prevalent issues: unemployment, juvenile crime, homelessness and lack of education. We help train and employ the disabled, seniors, at-risk youth and veterans in Maintenance, Construction, Landscaping, Custodial, Furniture/Assembly and Food Services.
Health Network
By partnering with human service agencies and health plans, care management is designed to connect individuals with the best possible treatment and provider for their mental health and/or substance abuse needs. Without this essential mediator, mental health and substance abuse care can often slip through the cracks of the traditional health care management system.
Health Services
AspenPointe Health Services assists with the mental health and substance abuse needs of children, youth, adults and families.
TeleCare
TeleCare's program provides confidential, professional and compassionate one-on-one telephonic outreach and ongoing support:
Depression Care Management: for people diagnosed with depression, or who have depression symptoms
Chronic Disease Management: for people with asthma, diabetes Type II or congestive heart failure who also have a mental health diagnosis
Recovery Care Management: for people in recovery from substance abuse.
