What is Depression Care Management?
In 2005, TeleCare developed and implemented a Depression Care Management (DCM) program, using a model based on the research of Dr. Kathryn Rost, to support people in managing their depression.  Depression can have an impact on every part of a person’s life, and can be related to physical illness and disease.
 
Who is eligible for the program?
Depression Care Management is often an employer-funded benefit, or may be offered through your health care plan. It is always a voluntary and confidential program that provides professional and compassionate one-to-one outreach and ongoing support. The program is clinically proven to help individuals feel better and to improve their overall health and well-being. How can Depression Care Management help me?
You will talk on the phone with a Depression Care Manager on a regular basis. Your Care Manager understands the serious problems that people with depression face. What might work for one person, may not work for another. Your Care Manager will customize your plan as you talk together to discover what’s effective for you.
 
What are the Care Manager’s qualificatons?
Care Managers are specially trained, licensed mental health professionals and registered nurses who will stay in touch with you by phone on a regular basis. Your Care Manager will provide you with caring and personal support. They are available to answer questions about depression, about medications and will support you each step you take toward your health and well-being.
 
Does the Depression Care Manager take the place of my personal physician or therapist?
No, your Care Manager works treatment plans with your health care providers. With your permission, your Care Manager will communicate with your doctor and your mental health provider about your progress to ensure that they have the important information they need to help you.
 
Will my employer find out if I enroll in the Depression Care Management program?
No, the DCM program is committed to providing clients with completely confidential services.TeleCare never provides employers a list of individuals who are eligible or who are enrolled in the DCM program. 
 
How do I get more information?
You can call 1-888-845-2881 or 719-572-6133 and ask to speak to a Depression Care Manager, or you
can email telecare@aspenpointe.org. A Care Manager will answer your questions and will help determine
your eligibility.

One of the largest nonprofits in Colorado Springs, AspenPointe traces its roots back to 1875 when we were called the Springs Relief Society. That organization, which delivered coal, lumber and clothing to the needy, was the genesis of what today is a comprehensive and integrated system of care that addresses the overall wellness of each client whether they need behavioral health services, counseling services or career services.

We have 12 organizations that serve more than 30,000 individuals and families each year through behavioral health services, counseling services, career services, education, housing, jail diversion/reintegration, telephonic wellness and care management.

We collaborate with clergy, educators, employers, other health and human service agencies, the legal/criminal justice system, other behavioral health providers, insurers, and many other organizations to leverage our collective resources in order to better our communities and the lives of its residents.

We are a 501(c)(3) nonprofit funded largely by Medicaid, state and federal grants, local government contracts, and generous donations.
