
Standard Case Management
Standard case management services are intended to provide additional supports to clients who are engaged in outpatient services. Possible case management referrals include: benefit procurement, assisting clients with housing or transportation services, attending community staffing, linkage with community supports, and assisting clients in learning daily living skills that will lead to more independent living.
Children
Case Management for children addresses skills development for children and their parents, assisting the child and family in developing a natural support system, and coordinating with external providers.
Intensive Case Management
Clients on the Comprehensive Services Team (Level IV) are served with a flexible model that includes drop-in as well as scheduled appointments for case management, medical services, home visits, community support, and individual and group therapy. A team of case managers helps clients (1) access community resources, (2) learn to manage symptoms of mental illness, and (3) set and achieve personal recovery goals.
